If you are interested in becoming a member of Dubois County CARES the process is simple!

1. Check out our Home page to see when our next coalition meeting is taking place and join us.

​

2. Please fill out the membership application located below. Paper copies are available at every coalition meeting. 

​

3. When the application is complete: 

        a. Drop the completed application off at the CARES' office, Room 8 in the Center on Fifth building (505 W. Fifth Street in Jasper)

                                               -OR-

        b. Mail the completed application to Dubois County CARES, 505 W. Fifth St., Jasper, IN, 47546

                                               -OR-

        c. Scan the completed application and email it to: DuboisCountyCARES@gmail.com 

     

       d. Bring the completed application with you to the next coalition meeting
